  • AHS Band Uniform Contract

    All students will be issued a marching band uniform / color guard spirit uniform for the school year. Each school owned uniform piece is numbered to help with inventory. Guidelines have been established for maintaining the quality of the uniforms. Grades will be affected as a result of improper uniform care.

    *School owned items that are issued from the uniform closet prior to each performance.

    • School owned uniform items are NOT TO BE TAKEN HOME!
    • Uniforms are to be worn properly for the duration of the event, performance, or activity.
    • Uniforms will be cleaned at the end of each season; cleaning fee is included in fees. Any excessive dirt or grime that requires additional cleaning will be charged to that individual student.
    • All students must remember that they are representing Arapahoe High School while in uniforms. No swearing or running, rough housing or other inappropriate behavior while in uniform will be acceptable.
    • No eating while in uniform.
    • The utmost respect and courtesy will be given to the ABB volunteers at all times.
    • No perfume, makeup, or hairspray is to be used while wearing marching jackets.
    • While not being worn, uniforms must be properly hung in the uniform bag that is issued to each student. Hats will be stored in the hat box provided.
    • At the end of each performance uniforms will be returned to the uniform closet. Each bag will be checked and checkin will not be allowed until all uniform parts are properly stored. No food or drink may be stored with uniforms.
    • Students are not permitted in the closet without supervision by staff or volunteers.
    • Rain ponchos will be issued in case of rain or snow.
    • Pep band tshirt must always be worn under the marching jacket. No camp shirts, trip shirts, sectional shirts or sleeveless shirts are allowed.
    • Students must be in complete uniform to perform. Students may NOT borrow items from the uniform closet.
    • In the interest of good hygiene, all student owned items must be taken home and washed. (socks and pep band shirts)
    • Students are responsible for keeping track of all parts of their uniform pieces including hat boxes and uniform bags. Students will be charged for missing or damaged items.
    • Replacement (forgotten) items are available for purchase in the uniform closet:
    • Uniforms should be worn with pride and respect. Any excessive damage or losses to a uniform will be the financial responsibility of that individual student. The student will be required to pay the replacement cost of the item damaged or lost.

    Initials below indicate acceptance of responsibility for the uniforms issued. It also indicates intent to keep the uniform at its best while in the student's possession.

  • Parent/Guardian email(s) provided in this form will be shared with the Arapahoe Boosters for sending student fee invoices, sharing band schedules and updates, information about uniforms and merchandise, and volunteer opportunities.
  • Parent/Guardian cell phone number(s) provided in this form will be shared with the Arapahoe Boosters for sending text messages from Arapahoe Band Boosters and staff regarding timely notifications of band activities or report times.
  • Student has my permission to go on all band or guard field trips on various dates throughout the school year.
  • Student has my permission to receive emails and text messages (via Remind) from Arapahoe Band Boosters and staff regarding band activities and share account updates.
  • Certain medical information (such as allergies, asthma, etc.) as well as emergency contact phone numbers will be shared with staff and chaperones.
  • Arapahoe Band Boosters request your permission to photograph or video your child for use on program related websites, communications and social media.
  • Student has my permission to ride in a private or commercial vehicle to an activity during the period June 1, 2022 through May 31, 2023. This is a school-related activity and will be supervised by his/her teacher. I understand that it is my responsibility to verify that the owner of the vehicle in which my son/daughter will be traveling has adequate liability insurance coverage. I understand that the district does not provide insurance coverage for personal or commercial vehicles used for school activities. Each participant is responsible to be at the site of the activity at the announced time. I further hereby release and forever discharge Littleton Public Schools, its representatives, employees, officers, and directors from any and all liability arising out of or relating to the private or commercial vehicle while transporting my child to any school-sponsored activity.
  • Medical Release

  • CONSENT FORM / RESPONSIBILITY CLAUSE / MEDICAL PERMIT
    I hereby agree that the above named student will participate in the Arapahoe High School Band Program. In case of emergency, I hereby give my consent for a qualified physician to perform any medical or surgical procedures s/he deems necessary to the welfare of this student while participating in the Arapahoe High School Band Program. It is understood that the Arapahoe High School and medical personnel will make every attempt to contact relatives listed above before taking any such actions. Further, this authorization permits said physician to hospitalize, secure appropriate consultation, order injections, anesthesia (local, general or both) or surgery for this applicant if such emergency conditions warrant. I, the undersigned, do hereby assume and agree to pay any indebtedness or physician’s or surgeon’s fees and hospital charges for such services.
  • NOTICE: ALL STUDENTS IN MARCHING BAND AND GUARD MUST ALSO HAVE THE PHYSICAL EXAMINATION FORM ON FILE WITH THE DIRECTOR OF BANDS NO LATER THAN AUGUST 1.

    NOTE: ALL FORMS FROM PREVIOUS YEARS ARE DISCARDED AT THE END OF MAY EACH YEAR. PHYSICAL EXAMINATION FORMS MUST BE VALID THROUGH THE ENTIRE SEASON (JUNE-NOVEMBER FOR MARCHING BAND)

    UPDATING FORMS IN A TIMELY MANNER IS THE RESPONSIBILITY OF EACH FAMILY

    Parent Permission for Student
    Passengers in Private Vehicles

  • (student) has my permission to ride in a private vehicle to an activity during the current school year. This is a school-related activity and will be supervised by his/her teacher. I understand that it is my responsibility to verify that the owner of the vehicle in which my son/daughter will be traveling has adequate liability insurance coverage. (Minimum standards for liability coverage are $100,000/$300,000 for bodily injury, $50,000 property damage).

    Each participant is responsible to be at the site of the activity at the announced time.

    I further hereby release and forever discharge Littleton Public Schools, its representatives, employees, officers, and directors from any and all liability arising out of or relating to the private vehicle while transporting my child to the school-sponsored activity.

    Please indicate your permission for your son/daughter to drive his/her private vehicle by filling out the required insurance carrier information and by signing and returning this form.

    I give my permission for my son/daughter to drive his/her/my own vehicle to school-sponsored events during the school year. I understand that all liability for any accidents will be borne entirely by my student and/or myself.

    Fees are for band camps, weekly student instruction, pep band shirt, show shirt, color guard costume, shoes, show props and accessories, and transportation.
